From that, we build a mechanistic model of your condition and an action plan, naming which specialists and centers of excellence to consider, and in what order. It comes with 30 days of follow-up. This is a living document you own and refer back to over time. Use it to understand and research your own condition, and to inform the conversations you have with your physicians.
